Medical Error is the 3rd leading cause of death in the US and diagnostic errors are the leading cause of medical malpractice claims in the emergency department. Since the To Err is Human report was published in 1999, more research has focused on the frequency and severity of diagnostic errors to patients, families, providers and the overall US healthcare system. Most recently, in January of 2021, the Leapfrog Group announced a new program to bring greater transparency and awareness to diagnostic errors across the US. During this presentation, attendees will gain insight into 1) the new research and importance of diagnostic errors to hospitals, 2) a set of clinical elements [DxDrivers] that when omitted have been shown to increase risk of diagnostic error and 3) methodologies that have been used to increase compliance with the DxDrivers using technology and decision support triggers.

Objectives:

LO 1. Attendees will be able to quantify the incidence of diagnostic error based on new research from Society to Improve Diagnosis in Medicine, Johns Hopkins and other leading academic authorities.

LO 2. Attendees will learn about the specific clinical elements that are often omitted and have historically been associated with diagnostic error, adverse events and malpractice litigation.

LO 3. Attendees will be able to identify methodologies for improving practitioner’s compliance with key clinical elements in the diagnostic process to improve patient safety, quality of care and reduction of adverse events.

This program follows the CPPS Content Outline as required for recertification by continuing professional education.  Content Code - 3 Type Code - A
